  • What do we assess?

    We complete an individualised assessment which helps us understand current challenges you have, how you are living your life and what is important to you currently and moving forwards into the future.


    Our assessments explore the following areas with you and those who know you best. 

    • Interests, values, expectations, hopes and goals
    • Daily/ weekly routines and balance of activities
    • Roles and responsibilities
    • Skills; physical, thinking/ cognitive, emotional, communication and interaction
    • What supporting factors are there currently?
    • What barriers and challenges exist for you? 
    • How well you are able to access and move around your home, local community, work, educational environments
    • Resources available to tap into
  • How do we assess?

    We visit you in your own home to complete an initial assessment which helps us find out about you and explores the information above. This is usually through an informal conversation with you and, if appropriate, someone who knows you well. It  may also involve standardised assessments or questionnaires which compare your responses to others'. 

    Observing how you complete agreed activities helps us to evaluate your skills in your home environment.

    We may also assess you in any other relevant environments you usually access; community, work, education.


  • What happens next?

    We write a report which summarises our assessment. This includes our recommendations, goals for treatment and projected costs. This is sent within two weeks to whoever made the referral to us to review and decide whether treatment can commence.
